The need worth working on.
Support often crosses providers, appointments, equipment and everyday relationships. People may repeatedly explain what helps them, while small communication failures create large barriers. This concept begins with the participant's chosen activity and preferred communication, including people whose needs are not obvious to an observer.
What the enterprise could offer.
Develop an accessible planning and coordination service that makes agreements, support preferences and next actions easy to understand. The first version could be a participant-controlled support passport and a practical task board. Any later AI assistant would help prepare and organise information, with human responsibility clearly assigned and participant permission governing sharing.

A participant-owned activity plan
Co-design the plan in plain language, screen-reader-friendly text and other formats the participants actually use.
Rehearse a handover between two supporters with fictional information before any live use.
Review whether the participant can change permissions, correct the plan and complete the activity with less repeated explanation.

A provider service or participant-selected support product could be explored after its actual funding and purchasing route is checked. Do not assume an expense is claimable.
